Arrange an appointment:
The surgery operates a ‘phone on the day’ appointment system for doctor appointments. A small number of pre-bookable appointments are available up to 2 weeks in advance for those who may find these more convenient. Telephone 01392 250135.

Request a repeat prescription:
There are 3 ways of organising a repeat prescription.
- Tick the required boxes on the repeat list attached to your prescription and either bringing this in or posting it to us.
- Writing your prescription requests down and giving them to the receptionists or putting the list in the post.

Request a home visit:
If you are too unwell to come to the surgery, the Doctor may visit you at home. Please telephone the surgery before 12 noon and give the Receptionist details of your condition. A doctor may ring you back to assess the problem before a visit.
Obtain a medical certificate:
For the first seven days absence from work a self-certification form should be completed. Such forms are available from your employer. If not, or you are self employed, you can obtain the appropriate forms from the reception desk. If a further certificate is needed after the first seven days you will need to make contact with your doctor. A telephone consultation may be sufficient.
Find out about my test results:
If you require the results of any tests or procedures that have been carried out, please telephone the surgery after 2.00 pm. Information will not be given to another person unless we have your written permission.
